The Mental Health Therapist provides clinical mental health services to assigned patients in the Adult Partial Hospitalization Program (Adult PHP), including Group, Individual, and Family Therapy, Admission and Discharge Assessments, Case Management, and Discharge Planning. The Mental Health Therapist also acts as a liaison between the Adult PHP and community providers and between the patient, family, and the Adult PHP team and assists other members of the treatment team in coordinating and implementing the patients’ individualized treatment plans

RESPONSIBILITIES

  • Completes psychosocial on assigned patients and assists treatment team in developing each patient’s individualized treatment plan.
  • Reassesses patient’s progress and develops discharge plan with treatment team by conducting interviews with patients, families, significant others, and outside agencies. Documents the same.
  • Facilitates program / treatment via individual, family, and group sessions.
  • Assist patients in coordination of social services in the community and / or transfers to other levels of care.
  • Assures confidentiality of all patient information and protection of patient’s rights.
  • Completes all documentation in a timely manner as required by HealthAlliance Hospital (HAHV) and MHS.
  • Participates in MHS and HAHV meetings as required including team meetings, case conferences, staff meetings, orientation, mandatory in service, staff development, and etc.
  • Alerts business office relevant to financial reimbursement issues and coordinates HMO contacts.
  • Maintains knowledge and proficiency and sees that planning and treatment are appropriate to age, specific developmental patterns, and health care needs.
  • All new hires will agree to cross train and float to other Mental Health units as needed.
  • All new hires will agree to provide on call services on a rotating basis following a three month orientation period.

QUALIFICATIONS/REQUIREMENTS

EXPERIENCE

  • Experience in mental health or related field required.

EDUCATION

  • Masters Degree in social work (MSW), Mental Health Counseling (MHC), Creative Art Therapist (CAT) or related field from an accredited program required.

LICENSES/CERTIFICATIONS

  • NYS Department of Education License or Limited Permit License to practice as an LMSW, LCSW, LMHC, or LCAT. Ongoing training in theory, principles, and modalities. Procedure and practice of mental health work and social services. Employee must maintain current licensure and/or required certifications.
