As a recognition of our commitment to quality and patient safety, we have received the highest distinction of Exemplary Standing from Accreditation Canada.Job SummaryThe Diagnostic Assessment Program (DAP) Nurse Navigator is responsible for guiding the patient through the entire diagnostic phase of cancer care. The DAP Nurse Navigator acts as an advocate for the patient in terms of facilitating access to timely diagnostics and treatment. The DAP Nurse Navigator helps to ensure that guidelines for high quality care are followed at every stage. The DAP Nurse Navigator regularly liaises with the appropriate physicians, radiologists, surgeons and Cancer Centre as needed to expedite the appropriate preoperative investigations.Qualifications
- R.N. certificate of registration from the College of Nurses of Ontario required; BScN preferred
- Minimum of 3-5 years of recent and relevant clinical experience oncology nursing required; CON (C) and thoracic or breast site experience preferred
- Minimum of 2 years’ nursing experience in a Diagnostic Imaging Department is preferred
- Successful completion of an approved Nurse Navigator Program within 1 year of hire (i.e. De Souza Institute)
- Demonstrated knowledge of the Central Regional Cancer Program and the broader cancer system within Ontario
- Demonstrated coaching, mentoring and team building skills that facilitate positive interactions among all members of the health care team.
- Possesses highly developed listening, analytical, problem solving and critical thinking skills.
- Demonstrated clinical decision-making and organizational skills.
- Demonstrated a strong patient and customer focused philosophy in all interactions.
- Possesses the ability to introduce and effectively facilitate change.
- Demonstrated an ability to provide leadership, support and critical analysis in stressful, complex clinical and emotional situations.
- Demonstrated an understanding of and experience with adult education strategies
- Demonstrated advanced therapeutic communication techniques and telephone support strategies
- Demonstrated a high degree of initiative and self-direction
- Demonstrated project planning and program development skills with a proven ability to present projects and presentations regionally and provincially
